What Is Community Engagement?
Community Engagement is a person-centered service that supports individuals in participating in meaningful activities in integrated community settings. Services are based on the individual's goals, interests, preferences, abilities, and Plan for Supports.
Person-Centered Goals
Our staff help individuals work toward meaningful outcomes identified in their Plan for Supports while encouraging choice, self-direction, independence, and community inclusion.
Transportation
Transportation may be provided or coordinated when appropriate to support authorized community-based activities and the individual's Plan for Supports. Availability is reviewed during the referral process.
